If your hosting is a Windows based ASP type of hosting you will need to contact your hosting company to see if they provide a sample contact form and support for form setup. You can also check in your hosting control panel to see if there are form setup or install instructions, or a sample form available.
Once you have a sample form, you can use the code from the sample to setup the contact.htm included with your template.
